Male infertility is a common and often overlooked part of the fertility journey. According to the American Society for Reproductive Medicine (ASRM), a male factor is involved in approximately 40-50% of infertility cases. Understanding male infertility is an important step toward timely diagnosis and effective treatment.
Male infertility most often stems from challenges with sperm production, sperm function, or sperm delivery. Problems identified during evaluation commonly include low sperm concentration, reduced sperm motility (movement), abnormal sperm morphology (shape), or in some cases, a complete absence of sperm in the ejaculate (azoospermia). These findings are typically identified through a semen analysis.
The causes of male infertility are varied. Hormonal imbalances affecting the testes or the pituitary gland can impair sperm production. A varicocele, an enlargement of veins within the scrotum, is one of the most common correctable causes. Genetic conditions, including Y-chromosome microdeletions or Klinefelter syndrome, may also play a role, particularly in cases of severe sperm abnormalities. Obstruction of the reproductive tract, prior infections, and certain medical treatments such as chemotherapy or radiation can impact fertility as well.
Additionally, lifestyle factors, including tobacco use, excessive alcohol intake, obesity, and environmental exposures, may negatively affect sperm quality. In some men, however, no clear cause is identified; this is referred to as idiopathic male infertility.
A comprehensive evaluation that includes a detailed medical history and at least one semen analysis is strongly recommended. If further evaluation is needed, hormonal testing, genetic testing, or imaging studies may be recommended.
In some cases, lifestyle modifications or medical therapy to address hormonal imbalances can improve sperm parameters. Surgical repair, such as correction of a varicocele or treatment of an obstruction, may be appropriate for select patients. When sperm counts are very low or absent, sperm retrieval procedures combined with assisted reproductive technologies can offer a path forward.
Male infertility is a medical condition and not a reflection of overall health.
